The notepad is for writing down any thoughts to remind you about the incense you just sampled. The score sheet is what you write down your answers on. This varies according to the Kumiko (Game).
This is the point where the host of the ceremony will give a talk and you will lectured on the History of Kodo, how to hold the Kiki-gouro, (incense cup) how to listen (smell) the incense, and you will be told about the six kinds of aloeswoods used.
